How to Refer a Patient
Specialists and General Practitioners are accredited to Hunters Hill Private Hospital, Centre for Surgical & Rehabilitation Specialities as Visiting Medical Officers and operate their practice in the local or surrounding areas. Pre Admission Policy
Please complete the pre admission form (link below)at the time of consultation. All relevant sections must be completed before giving to the patient. Please inform the patient that this form must be received at least 7 days prior to admission. If this is not possible or it they have any queries regarding cost, please ask them to telephone the hospital between 9.30am and 4pm Monday to Friday to make arrangements for their pre admission.

Referrals to Rehabilitation
Endeavour ward is a 26 rehab unit that provides rehabilitative care for Orthopaedic, Reconditioning, Neurosurgery and musculoskeletal patients.
We currently have 3 accepting consultants and a great multidisciplinary team including nursing, allied health, speech pathology and dietician, all working together to provide fantastic outcomes for our patients. The rehab unit is opened Monday to Saturday with Sunday a dedicated rest day.
For Referrals to the unit please download the Rehabilitation Referral Form and fax completed form to 9526 1702. The Nursing Unit Manager can be contacted on 9717 2342 or the After Hours Manager can be contacted on 9717 0182 with all enquiries.
The Nurse Unit Manager will get in touch to inform you of the patient's acceptance to the Rehab program and to inform you when a bed becomes available.
Day Rehabilitation Program is suitable if you have established rehabilitation needs but do not need the support of inpatient medical or nursing care. You may have stayed with us as an inpatient or have been referred to one of our Specialists from your GP. Joining our Day Rehabilitation program is easy. A request can be made by your specialist or GP.
